Magnesium Sulfate IFT (Bolus & Infusion)

Action:
CNS depressant, depresses smooth, skeletal and cardiac muscle

Indications:
Antiarrhythmic
Asthma
Preeclampsia /Eclampsia
Tocolysis

Precautions:
Hypersensitivity
Heart block
Impaired renal function
Hypocalcemia

Adult Dosing:

  • Asthma: 2g IV slowly (consider dilution in NS), repeat PRN
  • Eclampsia: Pregnancy > 20 weeks gestational age or post partum with seizures – Infuse 2-4 g slow IV push over 5-10 minutes the mix 4 gm, diluted in 50 ml of Normal Saline (0.9 NS), IV drip over 15-30 minutes as needed.
  • Antiarrhythmic (Torsades, Ventricular Fibrillation, Ventricular Tachycardia): 2 g IV, repeat q 3-5 minutes PRN
  • Pregnancy Induced Hypertension and Preterm Labor: Loading dose: 4 gm (10% solution) IV over 20 minutes. Maintenance: 2 gm / hour. (An additional bolus of Magnesium Sulfate can be administered only with receiving or sending OB physician order)

  • Asthma/antiarrhythmic– 25-50 mg/kg, IV or IO, slowly, Repeat PRN
